You may apply for a new U.S. Passport, and get renewal forms, at Hayward City Hall, City Clerk's Office - 4th Floor, 777 B St (at Watkins St) in Hayward, CA.

To apply at Hayward City Hall:
- Call to make an appointment - 510-583-4400.
- Complete a Passport Application or bring one with you.
Applicants under 17 years of age must appear in person with a parent or legal guardian. Minors, 15 and younger, require both parents to be present to indicate consent for passport issuance.
- Bring Proof of U.S. Citizenship.
For example, an original birth certificate.
- Bring Proof of Personal Identity.
For example, a California ID card.
- Bring two (2) identical, recent color Passport photos.
You may also get photos taken at the City Clerk's Office for $15 charge.
- Be prepared to pay for all applicable fees and optional postage charges at the time of application.
