U.S. Passports


You may apply for a new U.S. Passport, and get renewal forms, at Hayward City Hall, City Clerk's Office - 4th Floor, 777 B St (at Watkins St) in Hayward, CA.

A complete list of Passport Requirements is found on www.travel.state.gov along with forms, acceptable proof of identification and U.S. citizenship, and answers to most frequently asked questions.


To apply at Hayward City Hall:
  1. Call to make an appointment - 510-583-4400.

  2. Complete a Passport Application or bring one with you.
    Applicants under 17 years of age must appear in person with a parent or legal guardian. Minors, 15 and younger, require both parents to be present to indicate consent for passport issuance.

  3. Bring Proof of U.S. Citizenship.
    For example, an original birth certificate.

  4. Bring Proof of Personal Identity.
    For example, a California ID card.

  5. Bring two (2) identical, recent color Passport photos.
    You may also get photos taken at the City Clerk's Office for $15 charge.

  6. Be prepared to pay for all applicable fees and optional postage charges at the time of application.
